Job summary

FHE Health in Boca Raton is seeking a dedicated Case Manager to support patients through treatment, admissions, and discharge planning. This role acts as a liaison between patients, the clinical team, and admissions staff, ensuring a smooth care continuum.

You will conduct assessments, develop care plans, and coordinate access to therapy, medical care, and community resources while monitoring progress and providing ongoing support to help patients stay motivated and engaged in their treatment

Qualifications

• Knowledge of addiction and recovery preferred.
• Knowledge of CARF, DCF and Joint Commission Standards, preferred.
• Strong interpersonal and communication skills for working with patients and families.
• Knowledge of mental health conditions, treatment modalities and community resources.
• Prior experience in behavioral health or substance use treatment setting preferred.
• Current CPR Certification.

Responsibilities

• Conduct comprehensive assessments to identify clients' needs and develop individualized care plans.
• Coordinate and facilitate access to various services, including therapy, medical care, and community resources.
• Monitor clients' progress and adjust care plans as needed, ensuring effective treatment and support.
• Provide ongoing support and counseling to clients, helping them navigate challenges and maintain motivation.
• Collaborate with healthcare professionals, family members, and community organizations to ensure a cohesive support system.
