General Summary

Manage a designated caseload to coordinate and complete timely assessment, planning, implementation, and evaluation of discharge plans and care transitions across the continuum of care. Ensure optimum utilization of the patient's and the Health System's resources within CMS and other external review agencies' requirements.

Work Hours: Monday-Friday, some weekends and holidays included

Work Performed

  1. Monitor daily census and assignments to assure all patients are assessed for case management needs including care coordination, transition, discharge planning, consultations, advocacy, and education.
  2. Maintain effective communication with healthcare team members regarding assessment findings, discharge planning needs, and provider orders needed to arrange Homecare, Durable Medical Equipment, Transportation, Skilled Nursing, or Acute Rehab Facility placement, Substance Abuse Treatment, and outpatient follow-up.
  3. Maintain knowledge of specific benefits, reimbursement guidelines, the discharge planning process, and applicable federal, state, and local regulations. Provide education and guidance on these topics to providers, patients, and families as needed.
  4. Collaborate with Utilization Management partners to enhance negotiations and denial prevention with payers.
  5. Document assessment findings, discharge arrangements, and actions taken timely according to departmental guidelines; prepare reports and maintain records as required.
  6. Complete initial assessment within 72 hours to identify psychosocial strengths, weaknesses, community supports, and needs.
  7. Contact patients' primary support systems to gather information about support dynamics and needs.
  8. Conduct support system and family therapy to practice skills supporting community living and develop functional interaction patterns.
  9. Conduct group sessions for patients focused on skill-building and interactional skills.
  10. Assist in developing safety plans to promote positive coping strategies for community living.
  11. Facilitate discharge planning involving support systems to ensure safe community living environments.
  12. Interface with the team for multidisciplinary assessment of discharge needs and provide support system/family function information.
  13. Update Interqual as necessary.
  14. Perform other duties as assigned.

Knowledge, Skills, and Abilities

  • 3 years of adult/mental health experience preferred
  • Ability to work effectively in a self-directed role
  • Ability to multitask
  • Licensed Clinical Social Worker (LCSW)
  • Problem-solving skills for complex issues
  • Excellent written and verbal communication skills
  • Basic computer skills necessary

Minimum Qualifications

Education: Master's degree in social work from an accredited school

Experience: 3 years relevant experience

Licenses, Certifications: Current licensure as an LCSW by the NC Social Work Certification and Licensure Board; should also be a Notary.
