Responsibilities

• Identify in-need individuals and conduct intake and crisis intervention.
• Connect clients to services and resources.
• Explain processes and regulations surrounding eligibility and maintenance of services.
• Engage clients in developing individualized goals.
• Advocate on behalf of clients with other agencies/companies.
• Follow up on services to ensure participation and program compliance.
• Document all services in compliance with quality assurance plan.
• Report on program progress per funding guidelines.
• Participate in peer reviews and trainings.
