Qualifikationen

• Bachelor's degree in Social Work, Human Services, or similar health related field required.
• Minimum of 1 year of relevant experience preferred.
• NVCI within three months of hire required.

• Assist with patient education groups and facilitate discharge planning.
• Coordinate with the patient's support system and interdisciplinary team.
• Ensure safety and high-quality standards in discharge planning.
