Job summary

NovumHealth Summit County, Utah is seeking a Community Care Coordinator to help members navigate behavioral health and substance use services. You will coordinate care plans across providers, connect patients with treatments, and advocate for their needs in clinical and community settings.

This onsite role requires at least 1 year in case management or care coordination and is not eligible for remote work. Starting salary is $23 per hour with eligible benefits including mileage reimbursement,

• Strong knowledge of behavioral health systems, substance use treatment, and community resources.

Responsibilities

• Coordinate care plans across multiple providers and settings.
• Assist patients in navigating the healthcare system and accessing services.
• Schedule and coordinate appointments across therapy, psychiatry, MAT, and recovery programs.
• Document patient interactions and progress in electronic records.
