SUMMARY

The Care Navigator provides care coordination and case management services, supporting integrated and whole-person care for Behavioral Health clients. This role manages a defined roster of clients within an assigned level of care and supports access, continuity, and coordination of services across behavioral health, medical, and social service systems. Care Navigators serve on inter-disciplinary behavioral health care teams to engage and support clients, coordinate care across systems and providers, and address social determinants of health needs through advocacy, education, and resources.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Provides case management services to help clients engage in care and as defined in the service plan that coordinate, support, and advocate for clients who require assistance in gaining access to and using mental health, social, vocational, educational, child welfare, and other community services and resources. Provides an average of 6 hours of direct client service per workday to ensure community access to care and meet established performance standards.


• Screens clients for social determinants of health needs, mental health needs, substance use needs, suicide risk, and other needs, and engages clients or assists with referral to resources as needed.


• Provides case management and care coordination services that reflect person-centered and family-centered, recovery-oriented care, attending to the cultural and social needs of each client and family.


• Learns and implements evidence-based models of engagement, case management, and care coordination as determined by client need and PCH standards of care, which may include Motivational Interviewing (MI).


• Provides care coordination to engage and support clients, communicate and share knowledge and information, coordinate and support transitions of care, coordinate with internal and external care providers, and monitoring clients to adapt supports and treatment plans as needed.


• Tracks and follows client referrals to completion and “closes the loop” to facilitate and improve warm transitions and continuity of care.


• Tracks and supports client transitions across services and systems, including changes in level of care, service eligibility, and provider involvement.


• Ensures timely completion, maintenance, and updating of required consents and releases of information, including monitoring and addressing age-related consent changes (e.g., clients turning 12 or 18).


• Completes documentation in accordance with Behavioral Health and organizational policies and procedures, established timelines, and meeting quality standards.


• Works in collaboration with Care Teams across the organization (medical, dental, SA/DV), as indicated, to effectively serve client population.


• Coordinates with other community agencies and organizations such as provider organizations, schools, hospitals, and assistive resources.


• Provides mission-focused care and coordination towards improving the health of our communities and empowering individuals through compassionate, whole-person care. Embodies the values of PCH, including access, community, diversity, inclusivity, and respect, and service excellence, in all work operations.


• Participates in team meetings, staffing’s, and ongoing communications towards coordinated and collaborative care Behavioral Health clients.


• Participates in required meetings, in-service and other trainings, and consultations.


• Other duties as assigned.

EDUCATION AND EXPERIENCE

• Bachelor’s degree in psychology, social work or related field required. Equivalent combination of education and experience may be substituted in lieu of degree.


• Experience working with clients in a behavioral health setting is required.


• Case management and care coordination experience preferred.


• Attention to detail and capacity to work independently is essential.

LANGUAGE:

Bilingual (English/Spanish fluency) skills required

SCHEDULE AND PCH OFFICE LOCATIONS

This position will work at the Fillmore location in Berwyn and will spend at least 8 hours of coverage after 4pm and/or time including hours on Saturdays until 1 pm. At least 1 on-site in-person day will include hours until 8pm.

PAY & BENEFIT INFORMATION

$20.00 - $25.00 per hour*
