Job summary

Healthcare Alternative Systems (HAS) seeks to hire a Housing Case Manager to provide client-centered housing and case management services to individuals and families in the Chicago area.

Under the supervision of the Program Coordinator, you will link participants to housing opportunities, assist with searches, budgets, move-in plans, utilities, and connect to community resources.

Qualifications

  • Strong organizational, communication, and critical thinking skills.
  • Basic knowledge of computer keyboard.
  • Bilingual/Bicultural (English/Spanish), preferred.
  • Must be flexible with hours to accommodate participant needs.
  • Possession of a valid State Driver’s License and willingness to use personal vehicle.
  • HAS requires all staff to work on-site.

Responsibilities

  • Sources and creates linkage with community resources/landlords for housing opportunities.
  • Assists clients in housing search, including unit identification, viewings, applications, and lease signings.
  • Assists clients in establishing a housing budget.

  • Documents all service contacts and maintains case records per standards.
  • Collaborates with treatment team to improve participant outcomes.
  • Provides crisis management services as needed.
  • Maintains relationships with community agencies for referrals.

Healthcare Alternative Systems (HAS) provides a continuum of multicultural and bilingual (English/Spanish) behavioral care and social services that empower individuals, families, and communities. HAS seeks to implement sustainable programs that benefit society by improving the well-being and recovery efforts of those individuals, families, and communities impacted by behavioral health problems. With eleven locations across the Chicagoland area, HAS provides services and resources to individuals at every stage in their behavioral/recovery journey so that they can be successful and become contributing members of their communities.

JOB SUMMARY

Under the direct supervision of the Program Coordinator, the Housing Case Manager is responsible for offering comprehensive housing and case management services for a caseload of individuals and families. The Housing Case Manager provides client-centered and low-barrier care to participants and their households for the duration of their time in the program.

ESSENTIAL DUTIES
  • Manages a caseload of participants to ensure adequate services are being provided to assist participants with meeting their treatment needs.
  • Sources and creates linkage with the community resources/landlords to access housing opportunities for clients.
  • Assists clients in entire housing search process, including identifying appropriate units in communities of client’s choice, accompaniment to unit viewings, applications, and lease signings.
  • Assists clients in establishing a housing expenses budget.
  • Assists clients with move-in plan and furnishings within unit plan.
  • Assists clients in setting up utilities, as needed.
  • Links participants to needed and available community resources including legal, medical, housing, transportation, mental health, financial, etc.
  • Provides ongoing supportive and/or case management functions in accordance with the problems, needs, and the strategies identified within the service/treatment plan in order to help the participants to achieve the stated goals and objectives.
  • Provide crisis management services as needed to caseload, including for mental health crises, tenancy issues, and other types of client-identified concerns.
  • Connects participants to appropriate alternative housing resources to support successful transition from the program, depending on acuity and other available resources.
  • Documents all service contacts in a timely manner, including face-to-face interviews, collateral and networking contacts and correspondences. Maintains the case records in accordance with agency and regulatory standards and requirements.
  • Collaborates with the treatment team to ensure compliance and to work toward improved participant outcomes.
  • Provides anti-stigma education to the community and individuals dealing with mental illness and/or substance use disorders as well as information on prevention, treatment, and recovery for participants.
  • Meets regularly with the Program Coordinator as a means of enhancing professional growth, reviewing and processing the provision of case management services, and dealing with appropriate administrative issues.
  • Maintains a networking relationship with community agencies, clinics and supportive services for referrals/linkages and ancillary services.
  • Provides specific case management support, services, and documentation associated with any private or public grants.
  • Performs other duties as assigned by the Program Coordinator.
QualificationsREQUIRED SKILLS/ABILITIES

  • 100/300/100 Automobile Insurance.
  • HAS requires all staff to work on-site.
EDUCATION & EXPERIENCE
  • GED/High School Diploma.
  • Have two (2) years of case management experience in a non-profit setting.
  • Knowledge of housing and homeless services in Chicago is strongly preferred.
PHYSICAL REQUIREMENTS

Sedentary work. Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. Repetitive motion. Substantial movements (motions) of the wrists, hands, and/or fingers. The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading.Employees must be able to perform the essential functions of the position satisfactorily and, if requested, reasonable accommodations will be made to enable employees with disabilities to perform the essential functions of their job, absent undue hardship.
