Spero is seeking a Care Coordinator to integrate and support person-centered services for individuals receiving care through the CCBHC. You will work with clients, families, and providers to ensure timely access to services and resources.
As a Care Coordinator, you will navigate behavioral health, medical, social, and community services while promoting engagement, continuity, and better outcomes. This role follows a hybrid schedule with in-office and remote work.

The CCBHC Care Coordinator is responsible for coordinating and supporting integrated, person-centered services for individuals receiving care through the Certified Community Behavioral Health Clinic (CCBHC). This position works collaboratively with clients, families, behavioral health providers, primary care providers, social service agencies, and community partners to ensure individuals have timely access to appropriate services and resources.
The Care Coordinator serves as a consistent point of contact for clients and helps facilitate communication across providers and systems. The position supports individuals in navigating behavioral health, medical, social, and community-based services while promoting engagement in care, continuity of services, and improved outcomes.

Hear and speak in order to use telephones and communicate with people inside and outside the office.
Physical ability to move around the office area and in the community throughout each workday.
Visual ability to review detailed reports and information, and to use the computer for keyboarding.
Bend at the waist or crouch for filing charts in the filing cabinets.
Physical ability to Exert up to 20 pounds or up to 10 pounds of force frequently.
This position operates on a hybrid schedule, requiring two days in office and offering 3 days of remote work flexibility. Candidate will work within an integrated, multidisciplinary team environment, requiring frequent coordination with medical, behavioral health, and social service professionals to ensure holistic client care.
Daily local travel may be required; candidates must have the physical ability to operate a motor vehicle safely and must possess a valid driver’s license.
