Job summary

Centene is hiring a licensed behavioral health professional to develop and facilitate complex care management for members with mental health and substance use needs, including personalized care plans and education for families.

This role requires California licensure, 2–4 years of experience, and the ability to work PST hours, with flexible remote, hybrid, field, or office arrangements. You will coordinate with providers, monitor plans, educate members, and ensure regulatory compliance.

Qualifications

• Requires a Master’s degree in Behavioral Health or Social Work or a Degree from an Accredited School of Nursing.
• 2–4 years of related experience.

Responsibilities

• Evaluates member needs via phone or in-home visits related to resources and develops care/service plans.
• Facilitates care coordination to ensure timely access to needed care or services.
• Monitors care plans and provides recommendations based on member needs.
• Provides education to members and families on procedures, healthcare options, referrals, and benefits.
• Ensures compliance with state, federal, and payer regulations.
