Optimal Care, Inc. seeks a Behavioral Health Care Manager to integrate mental health support into primary care in Jackson, MI. You will manage a defined caseload, perform screenings, and provide brief evidence-based interventions as part of a collaborative care team with physicians and psychiatrists.

The role requires travel within the Jackson service area to patients' homes, assisted living facilities, or nursing homes. Hours are typically Monday to Friday, 8 a.m.

Qualifications

• Formal education or specialized training in behavioral health (social work, nursing, or psychology)
• Minimum 2 years of social work or behavioral health experience
• Experience screening for common mental health and substance use disorders
• Reliable transportation, valid driver’s license, and valid automobile insurance coverage

Responsabilités

• Integrate behavioral health into primary care settings and manage a defined patient caseload.
• Conduct screenings and provide brief evidence-based interventions as part of a collaborative care team.
• Track treatment progress and ensure patients stay engaged in care.
• Coordinate care with physicians, psychiatrists, and other behavioral health providers; support care transitions and resource connections.
• Travel daily within the Jackson service area to provide care to patients in homes, assisted living facilities, or nursing homes.
