Job summary

Molina Healthcare is seeking a Care Coordination Specialist to support adults with complex behavioral health needs in New York. You will conduct comprehensive assessments, develop integrated care plans, and coordinate with a multidisciplinary team to improve outcomes and reduce gaps in care.

Travel to member locations may be required, and the role emphasizes trauma-informed care, social determinants of health, and engagement strategies to enhance treatment adherence.

Qualifications

• At least 2 years health care experience, preferably in behavioral health.
• Licensed behavioral health clinician with active license.
• Valid driver\'s license and reliable transportation.
• Experience with whole person care principles and care coordination.

Responsibilities

• Completes comprehensive behavioral health assessments per timelines and determines eligibility for care coordination/case management.
• Develops and implements care plans with member, caregiver, physician and care team to address needs.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness, documents interventions and outcomes.
• Maintains ongoing member caseload for outreach and management.
