Job summary

Lifekind Health in Indio, CA is hiring a Behavioral Health Care Manager to bridge behavioral health, medical services, and care coordination. You will support Enhanced Care Management and work with two clinics (Rancho Mirage and Indio) to improve engagement and outcomes.

The ideal candidate holds an MSW or MFT (unlicensed) with APCC/ASW/AMFT certification or equivalent, and has CPR/BLS certification. Travel between clinics is required.

Qualifications

• Master’s degree in Social Work (MSW) or MFT (unlicensed) or related field with APCC/ASW/AMFT certification.
• BLS or CPR certification required.
• Experience in behavioral health, care coordination, or case management preferred.
• Ability to travel to the two clinics in the assigned region.

Responsibilities

• Provide ECM-aligned care coordination for high-acuity patients.
• Conduct proactive patient outreach, engagement, and follow-up.
• Identify and address barriers to care (transportation, psychosocial factors).
• Monitor behavioral health status and escalate clinical concerns as needed.
• Facilitate communication across interdisciplinary care teams.
• Ensure accurate, timely, and compliant documentation per payer requirements.
• Support continuity of care across behavioral health, medical, and ancillary services.

Salary Range: $72,000.00 To $82,000.00 Annually

Lifekind Health is looking for a full-time Behavioral Health Care Manager to serve as a critical link between behavioral health, medical services, and care coordination teams. This role supports compliance with Enhanced Care Management (ECM) program requirements while improving patient engagement, continuity of care, and clinical outcomes for moderate to high-risk populations. Help to increase show rates, ensure compliance, and optimize clinical staff efficiency by shifting non-billable tasks away from licensed providers.

About the position:

• Provide Enhanced Care Management (ECM)-aligned care coordination for high-acuity patients and patients with comorbid behavioral and medical conditions
• Conduct proactive patient outreach, engagement, and follow-up
• Identify and address barriers to care (e.g., transportation, psychosocial factors)
• Monitor behavioral health status and accelerate clinical concerns appropriately
• Facilitate communication across interdisciplinary care teams
• Ensure accurate, timely, and compliant documentation per payer requirements
• Support continuity of care across behavioral health, medical, and ancillary services

Our Ideal Candidate will have the following qualifications:

• Master’s degree in Social Work (MSW) or Marriage and Family Therapy (MFT) (unlicensed); or Master’s in a related mental health field with an Associate Professional Clinical Counselor (APCC), Associate Clinical Social Worker (ASW), or Associate Marriage & Family Therapist (AMFT) certification
• Basic Life Support (BLS) or CPR certification required
• Experience in behavioral health, care coordination, or case management strongly preferred
• Strong interpersonal and communication skills
• Ability to work within a multidisciplinary care model
• Knowledge of Medi-Cal and ECM program requirements preferred
• Ability and willingness to travel to the 2 clinics in your assigned region
