Job summary

Lifekind Health, Inc. in San Jacinto, CA is seeking a full-time Patient Liaison to bridge gap between patients and healthcare providers. You will coordinate appointments, explain treatment plans, assist with insurance and billing questions, and support patients throughout the care journey.

This role requires excellent communication, HIPAA knowledge, and a patient-centered approach to improve access and satisfaction. Strong EHR and scheduling tool experience is preferred.

Qualifications

• High school diploma or equivalent required; higher degrees preferred in healthcare or related field.
• Healthcare setting or patient advocacy experience is a plus.
• Ability to understand healthcare terminology and basic procedures.

Responsibilities

• Serve as the primary contact for patients, addressing inquiries and providing information about services and procedures.
• Coordinate scheduling of appointments, follow-ups, and referrals for seamless access to care.
• Collaborate with medical staff to clarify treatment plans and explain options to patients.
• Assist patients with navigating insurance processes, billing questions, and financial assistance.
• Document patient interactions accurately and maintain confidentiality per regulations.
• Identify and escalate patient concerns to appropriate departments for resolution.
• Provide emotional support and resources to patients and families.

Salary Range: $60,000.00 To $70,000.00 Annually

Lifekind Health is looking for a full time Patient Liaison in our San Jacinto clinic to serve as a vital bridge between patients and healthcare providers, ensuring clear communication and a supportive experience throughout the care journey. Understand patient needs, address concerns, and facilitate access to medical services and resources. Work closely with clinical teams to coordinate appointments, explain treatment plans, and assist with administrative processes, thereby enhancing patient satisfaction and outcomes. About the position:

• Serve as the primary point of contact for patients, addressing inquiries and providing timely information about healthcare services and procedures
• Coordinate scheduling of appointments, follow-ups, and referrals to ensure seamless patient access to care
• Collaborate with medical staff to clarify treatment plans and assist patients in understanding their healthcare options
• Assist patients with navigating insurance processes, billing questions, and financial assistance programs as needed
• Document patient interactions accurately and maintain confidentiality in compliance with healthcare regulations
• Identify and elevate patient concerns or complaints to appropriate departments to facilitate resolution
• Provide emotional support and resources to patients and their families, fostering a compassionate care environment

Our Ideal Candidate will have the following qualifications:

• High school diploma or equivalent required; Associate's or Bachelor's degree in healthcare, social work, nursing, or related field preferred
• Previous experience in a healthcare setting, customer service, or patient advocacy role
• Strong understanding of healthcare terminology and basic medical procedures
• Excellent communication and interpersonal skills to effectively interact with diverse patient populations
• Experience working with electronic health record (EHR) systems and scheduling software
• Ability to maintain patient confidentiality and comply with HIPAA regulations
• Bilingual abilities to support non-English speaking patients strongly preferred
• Training or certification in patient advocacy or healthcare navigation a plus
