Lifekind Health in Indio, CA is seeking a full-time Behavioral Health Care Manager (ASW/AMFT/APCC) to connect behavioral health with medical services and care coordination. The role supports ECM adherence, improves engagement, and optimizes outcomes for moderate to high-risk patients.

Two clinic locations—Rancho Mirage and Indio—provide care coordination across teams. The ideal candidate holds a MSW/MFT (or related field with required certificates), excels in teamwork, and is willing to travel

Qualifications

• Master’s degree in Social Work (MSW) or Marriage and Family Therapy (MFT) (unlicensed) or related field with APCC/ASW/AMFT certification.
• Experience in behavioral health, care coordination, or case management preferred.
• Strong interpersonal and communication skills.
• Ability to work within a multidisciplinary care model.
• Knowledge of Medi-Cal and ECM program requirements preferred.
• Willingness to travel to two clinics in assigned region.

Responsibilities

• Provide ECM-aligned care coordination for high-acuity patients and those with complex behavioral and medical needs.
• Conduct proactive patient outreach, engagement, and follow-up.
• Identify and address barriers to care (transportation, psychosocial factors).
• Monitor behavioral health status and elevate concerns as needed.
• Facilitate communication across interdisciplinary care teams.
• Ensure accurate, timely, and compliant documentation per payer requirements.
• Support continuity of care across behavioral health, medical, and ancillary services.

Lifekind Health is looking for a full-time Behavioral Health Care Manager to serve as a critical link between behavioral health, medical services, and care coordination teams. This role supports compliance with Enhanced Care Management (ECM) program requirements while improving patient engagement, continuity of care, and clinical outcomes for moderate to high-risk populations. Help to increase show rates, ensure compliance, and optimize clinical staff efficiency by shifting non-billable tasks away from licensed providers.

About the position:

• Conduct proactive patient outreach, engagement, and follow-up
• Identify and address barriers to care (e.g., transportation, psychosocial factors)
• Monitor behavioral health status and elevate clinical concerns appropriately
• Facilitate communication across interdisciplinary care teams
• Ensure accurate, timely, and compliant documentation per payer requirements
• Support continuity of care across behavioral health, medical, and ancillary services

Our Ideal Candidate will have the following qualifications:

• Master’s degree in Social Work (MSW) or Marriage and Family Therapy (MFT) (unlicensed); or Master’s in a related mental health field with an Associate Professional Clinical Counselor (APCC), Associate Clinical Social Worker (ASW), or Associate Marriage & Family Therapist (AMFT) certification
• Experience in behavioral health, care coordination, or case management strongly preferred
• Strong interpersonal and communication skills
• Ability to work within a multidisciplinary care model
• Knowledge of Medi-Cal and ECM program requirements preferred
• Ability and willingness to travel to the 2 clinics in you assigned region
