This position is a fully remote position supporting Sunshine Health Plan. Applicants must reside in Florida and hold licensure as one of the following:

  • LMFT
  • LMHC
  • LCSW

Position Purpose: Develops, assesses, and facilitates complex care management activities for members with mental and behavioral health needs to provide high-quality, cost-effective healthcare outcomes. Responsibilities include creating personalized care plans, member and family education related to mental health and substance use disorder.

  1. Evaluates member needs via phone or in-home visits, recommends and facilitates care/service plans, including behavioral health and social determinants.
  2. Performs outreach through telephonic, digital, home, or other site visits to assess needs and collaborate with resources.
  3. Develops ongoing care plans for high-acuity members, identifying providers, specialists, and community resources.
  4. Coordinates between members, families/caregivers, community resources, and care teams to ensure access to services.
  5. Monitors care plans and member outcomes, adjusting as needed based on clinical changes.
  6. Facilitates care coordination ensuring timely access to care and services.
  7. Maintains accurate documentation complying with regulations.
  8. Provides education to members and families on procedures, treatment options, and healthcare benefits.
  9. Offers feedback to leadership for quality improvement.
  10. Performs other duties as assigned and complies with policies.

Education/Experience: Master's degree in Behavioral Health, Social Work, or Nursing from an accredited school, with 2-4 years of related experience.

License/Certification:

  • Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN with BH experience.
  • For Sunshine Health (FL): Master’s in social work or related field, with one year of pediatric care experience. May require up to 80% local travel.
