The Behavioral Health Care Coordinator (BHCC) works as a key member of the Integrated Behavioral Health (IBH) Team and collaborates with the Interdisciplinary Teams (IDT) for the region as part of InnovAge’s Program of All-Inclusive Care for the Elderly (PACE). Our IBH team functions largely as a Consultation Liaison service and includes licensed therapists, Psychiatric Mental Health Nurse Practitioners (PMHNP), and external behavioral health providers.
The BHCC coordinates behavioral health referrals, supports patient panels, manages assessments and interventions, and serves as a resource across centers, assisting with onboarding, education, data tracking, scheduling, and meetings.
Required: Bachelor's degree in health/human services; enrollment in a master’s licensure program; 2+ years in mental health; experience with Electronic Health Records and Microsoft Office; valid driver’s license and personal transportation.
Preferred: Additional training, experience with older adults, EPIC EMR familiarity.
