Responsibilities

The Behavioral Health Care Coordinator (BHCC) works as a key member of the Integrated Behavioral Health (IBH) Team and collaborates with the Interdisciplinary Teams (IDT) for the region as part of InnovAge’s Program of All-Inclusive Care for the Elderly (PACE). Our IBH team functions largely as a Consultation Liaison service and includes licensed therapists, Psychiatric Mental Health Nurse Practitioners (PMHNP), and external behavioral health providers.

The BHCC coordinates behavioral health referrals, supports patient panels, manages assessments and interventions, and serves as a resource across centers, assisting with onboarding, education, data tracking, scheduling, and meetings.

Essential Functions and Responsibilities

• Provides direct support to participants through check-ins and screeners.
• As a non-licensed teammate, interacts with participants for screening, active listening, validation, motivational interviewing, safety planning, and psychoeducation under supervision.
• Re-escalates participants with worsening symptoms to licensed clinicians.

Coordination of Care

• Fields referrals, collaborates with providers, and facilitates appointments.
• Coordinates outside referrals and relays medical records.

IDT Collaboration

• Participates in case consultations and care coordination.
• Supports crisis de-escalation and stabilization.

Administrative Duties

• Maintains accurate documentation and manages data related to referrals, panels, and initiatives.
• Supports regional behavioral health activities.

Travel Requirements

• Travel between local worksites, to homes, and out of state, each 10% or less.

Qualifications

Required: Bachelor's degree in health/human services; enrollment in a master’s licensure program; 2+ years in mental health; experience with Electronic Health Records and Microsoft Office; valid driver’s license and personal transportation.

Preferred: Additional training, experience with older adults, EPIC EMR familiarity.

Salary Range

$26.88-$33.61 per hour
