Job summary

The Centers is seeking a Behavioral Health Specialist in Cleveland, OH to provide care coordination and integrated behavioral health services within a clinical team. The role links clients to health promotion activities and community resources while maintaining therapeutic relationships.

Responsibilities include assessing care needs, coordinating with medical teams, and supporting clients across social determinants of health. Strong collaboration and communication are essential.

Qualifications

• Bachelor’s degree in Social Work, Psychology or a mental health related field.
• Valid driver’s license in Ohio with access to insured vehicle.
• Ability and desire to work closely with other team members.
• Ability to form effective therapeutic relationships to engage clients and coordinate care.
• Excellent oral and written communication skills.

Responsibilities

• Provide community psychiatric support and care coordination including health promotion, transitional care, and advocacy.
• Conduct level of care assessments.
• Support clients with social determinants of health such as housing, social supports, economic and education needs.
• Monitor client status and service utilization to maintain appropriate level of care.
• Develop individualized treatment plans and perform reviews.
• Communicate vital treatment information to coordinate care with the centers team.
• Collaborate with nurses, physicians, and pharmacists to improve outcomes.
• Assist clients in obtaining and maintaining comprehensive healthcare.
• Attend medical appointments with clients and assist with transportation.
