Qualifications

Responsibilities

• Ensures the wellbeing of patients and provides a safe, positive, supportive, and structured environment under direction of charge nurse or supervisor.
• Assists patients with activities of daily living such as toileting, bathing, dressing, grooming, oral hygiene, meals, hydration and changing bed linens.
• Conducts safety checks and ensures supervision intervals per guidelines.
• Checks patients for contraband and manages valuables and belongings.
• Interacts with patients, observes behaviors, and communicates observations to nursing staff.
• Engages with patients in activities to achieve treatment goals.
• Facilitates patient educational and rehabilitative groups covering social skills, coping skills, anger management, and independent living.
• Takes and records vital signs and communicates significant changes to the nurse in charge.
• Charts in patient records timely and accurately.
• Applies de-escalation techniques and assists with restraints when necessary.
• Assists with unit cleaning, admits/discharge processes, filing, and incident reports.
• Provides transfer or transportation for patients as required and briefs oncoming shift.
