Job summary

Carrollton Springs, part of Lifepoint Health, is seeking an Assessment Specialist (PRN) to support admissions, initial assessments and care coordination for adult and geriatric patients in Carrollton, TX.

You will contribute to determining level of care, ensure accurate documentation, and collaborate with physicians for timely decisions while upholding compliant preadmission processes and follow-up activities.

Qualifications

• Master's degree in Social Work, Counseling or equivalent.
• Experience in a psychiatric healthcare facility with adult and geriatric patients preferred.
• CPR and de-escalation certification required or obtained within 30 days of hire.
• Current clinical, social work or RN license as required by state regulations.

Responsibilities

• Facilitates response to requests for services, including the initial response, assessment, and referral process, and designation of level of care.
• Documents patient symptomology, functioning, and needs and provides information to MD for LOC determination.
• Maintains preadmission and admission paperwork with accuracy and coordinates with insurance for pre-certification.
• Demonstrates excellent customer service with patients, physicians, visitors, and coworkers in a prompt, courteous manner.
