Job summary

True North Recovery Inc. is seeking a licensed counseling professional to perform comprehensive assessments and coordinates EHRs in an outpatient setting.

The role involves diagnosing substance use and mental health disorders, coordinating referrals, and ensuring timely documentation within the program. The ideal candidate holds a Master’s degree in counseling or related field, with experience in substance use/mental health and a willingness to obtain CDC II within 3 years of hire.

Qualifications

• Master's degree in counseling or related field.
• CDC II or higher preferred; BHC II or combination of experience, credentials and education.
• Must be able to obtain CDC II within 3 years of hire.
• Experience in conducting assessments is preferred.
• Experience and knowledge in substance abuse and mental health field.

Responsibilities

• Completes integrated substance use and mental health assessments using DSM-5-TR.
• Reviews assessments with supervisor if not licensed/experienced to diagnose.
• Recommends placement into treatment based on level of care after assessment.
• Notifies referral source of assessment outcomes and treatment recommendations.
• Maintains confidential electronic health records for each client.
• Adheres to documentation deadlines and program policies.
• Assists with intake documentation and maintains in EHR.
• Orientates clients to facility rules and hours.
• Gathers information for diagnosis, prognosis and treatment recommendations.
• Coordinates service delivery and advocates for clients to access resources.
