Job summary

True North Recovery is seeking a licensed clinician to perform integrated substance use and mental health assessments in an outpatient setting, diagnosing with DSM-5-TR criteria and coordinating electronic health records and referrals.

The ideal candidate holds a Master’s degree in counseling or a related field and CDC II or higher preferred; relevant experience in substance abuse and mental health is required. This is a full-time role with comprehensive benefits.

Qualifications

  • Master's degree in counseling or related field.
  • CDC II or higher preferred; BHC II or combination of experience, credentials and education.
  • Must be able to obtain CDC II within 3 years of hire.
  • Experience in conducting assessments is preferred.
  • Experience and knowledge in substance abuse and mental health field.

Job Description

Performs comprehensive clinical assessments, establishes diagnostic criteria for substance use and mental health disorders, and coordinates electronic health records and referral services within an outpatient setting.

Roles and Responsibilities
  • Completes integrated substance use / mental health assessments, and diagnoses substance use and mental health disorders using the DSM-5-TR criteria.
  • Ensures all diagnostic assessments are reviewed by the appropriate supervisor if the assessment counselor does not have an independent license and/or experience to diagnose.
  • Recommends placement into substance abuse treatment based upon level of care after completion of the integrated substance use / mental health assessment.
  • Informs referral source (with required release of information) of the outcome of the assessment and treatment recommendations.
  • Prepares and maintains a confidential, accurate, organized, and updated electronic health record for each assigned client under appropriate supervision.
  • Adheres to documentation deadlines as outlined in the Training Manual.
  • Complies with documentation requirements as outlined in program policies, protocols, and procedures for activities such as assessments, treatment plans, admission level of care (loc), and in the preparation of required reports.
  • Assists with intake documentation as necessary (e.g., consent for treatment, consents to release of information, initial assessment) and maintains same in the electronic health record.
  • Effectively orients the client to the facility/program(s) as needed by providing information including, but not limited to: rule infractions that may result in termination, the hours of service.
  • Uses the appropriate forms and instrumentation to assess client’s strengths and weaknesses.
  • Gathers relevant information pertaining to diagnosis, prognosis, and treatment recommendations.
  • Coordinates the delivery of service provisions. Acts as an advocate in accessing other needed services.
  • Becomes proficient and adheres to True North Recovery’s treatment philosophy.
  • Maintains familiarity with current and available community resources and their limitations, the procedures for making referrals, and the confidentiality requirements inherent to the referral process so that community resources are used to benefit clients.
  • Recognizes the need for case consultation with supervisors by participating in case reviews, maintaining contact with referring professionals, and seeking assistance in matters that exceed the counselor’s level of expertise and/or scope of practice.
  • Identifies program needs and suggests improvements to the Assessment Supervisor and Executive Leadership.
  • Manages time and organizes work demands appropriately.

Physical Requirements

This role is primarily performed in an office building with a standard office environment and controlled temperatures. When working outdoors or at a job site the employee may be exposed to construction equipment or other moving objects/equipment; reference job site specifics. The employee is occasionally required to lift and/or move up to 50 lbs. Employee is frequently required to sit, stand, walk, talk/hear, see, read, and personally interact with others.
