Job summary

LiveWell Homecare Agency in Georgia seeks a licensed Behavioral Health Assessor to perform comprehensive assessments, psycho-education, and individualized treatment planning for patients with behavioral health, trauma, or substance-related needs. You will coordinate with care teams and support discharge planning.

Responsibilities include face-to-face and tele-mental health assessments in acute settings, suicide risk evaluation, and documentation in EMR.

Qualifications

• MSW or master's in professional counseling from an accredited program.
• Licenses including BLS, Workplace Violence Prevention, and GA licensure as a Licensed Associate Professional Counselor or Licensed Master Social Worker.
• Minimum of 3 years’ experience working with adult/child populations, preferably behavioral health.
• Proficiency in Word, Excel, and telemedicine technology.
• Excellent verbal and written communication; ability to think abstractly.

Role Overview: The Behavioral Health Assessor-Licensed performs comprehensive assessments, psycho-education, and interventions for patients with psychological, behavioral, or substance-related issues. Working under the guidance of the Manager of Behavioral Health, this role involves collaborating with care teams to develop safe, individualized treatment plans and ensuring proper discharge and placement.
