The Behavioral Health Assessor-Licensed performs comprehensive face-to-face and tele-mental health assessments, psycho-education, and interventions for patients with psychological, behavioral, or substance-related disturbances. Working under the supervision of the Manager of Behavioral Health, they collaborate with physicians and care teams to develop safe, individualized treatment plans and coordinate patient care transitions. Key responsibilities include conducting assessments, including suicide risk evaluations, and providing care recommendations. They assist with discharge planning, legal documentation, and referrals to appropriate levels of care, involving patient families and support systems. The role requires documenting assessments and care plans accurately, participating in service line initiatives, and adhering to ethical standards, confidentiality, and professional development.
A Master’s degree in Social Work or professional counseling from an accredited program, with licensure as a Licensed Associate Professional Counselor or Licensed Master Social Worker in Georgia. A minimum of 3 years of experience working with adult/child populations in behavioral health is required. Skills include effective communication, innovative thinking, knowledge of behavioral health best practices, and basic technological proficiency, including telemedicine tools.
