Job summary

Rosecrance is seeking a Mental Health Professional to provide comprehensive diagnostic assessments and determine service needs. You will develop individualized care plans, coordinate with staff, and connect clients to appropriate community resources for recovery.

Strong documentation and collaboration skills are essential for success. Qualified candidates will have relevant degrees, licensure, and experience with DSM criteria.

Qualifications

• Bachelor’s in counseling, psychology, social work or related field. Master’s degree strongly preferred.
• Must qualify as an MHP or higher per Illinois DMH Rule 132.
• CADC or licensed in Illinois as LSW/LPC or higher within 2 years of employment.
• Knowledge of DSM diagnoses and criteria.
• Knowledge of assessment techniques.
• Ability to establish rapport quickly.
• Computer proficiency required; proficient in documentation in EHR.
• Ability to complete required documentation within time frames.
• Self-starter with ability to work independently and in a team.

Responsibilities

• Provide centralized diagnostic assessments including social histories and mental status.
• Determine level of care and develop ISP with clients.
• Identify need for crisis intervention services.
• Maintain documentation per QA procedures and standards.
• Assess needs and develop comprehensive care plan.
• Linkage and referrals with community resources.
• Collaborate with work team to achieve objectives.
• Educate community agencies about Rosecrance services.
• Participate in Access team meetings and activities.
