Job summary

Horizon House, Inc. is seeking a Behavioral Health Clinician to provide individual and group outpatient counseling within our Behavioral Health Services Division. You will monitor an assigned caseload, develop recovery plans, and support clients toward a substance-free lifestyle in a compliant, compassionate environment.

The role emphasizes collaboration with care teams, documentation accuracy, and engagement in treatment planning to address diverse client needs.

Qualifications

• Master's degree in a relevant field is required or equivalent RN with mental health nursing certification.
• CADC preferred; if not certified, must complete full certification within 5 years of hire.

Responsibilities

• Provide individual, group, and/or family therapy according to each client's recovery plan.
• Complete and maintain chart documentation per insurance, licensing, and Medicaid standards.
• Ensure client input into recovery plans and modify as needed.
• Coordinate with care managers, medical staff, and family members to provide ongoing support.

Job description

We are seeking a Behavioral Health Clinician to work in our Behavioral Health Services Division, providing support, education, counseling, and guidance to clients in recovery, and developing and reinforcingmechanisms and resources to support a substance-free lifestyle. Ensure a safe, secure, and pleasant environment, in compliance with Agency policies and procedures and applicable regulations. Implement Horizon House's mission, values, and beliefs in all aspects of the job. Monitor and track the progress of the assigned caseload. Provide individual and group outpatient counseling services for an assigned caseload of clients.

Other Responsibilities (not all-inclusive)

• Provide individual, group, and/or family therapy according to each individual's recovery plan.
• Complete and maintain all chart documentation in compliance with insurance, licensing, and Medicaid Certification standards, including progress notes, recovery plans, and clinical reviews within established timeframes.
• Ensure each client has input into developing an up-to-date recovery plan; modify plans to adapt to special needs and/or service delivery gaps.
• Establish an ongoing dialogue with other members of the rehabilitation/treatment team, including care managers, vocational/educational staff, consultants, members of the medical profession, and family members, as needed, to provide continuous support; schedule planning and linking conferences as needed

Qualifications:

• Master's degree in Psychology, Rehabilitation Counseling, Social Work, Nursing, or related field from an accredited college or university (or a registered nurse with a certificate in mental health nursing from the American Nurses Association). Certified Alcohol and Drug Counselor (CADC) preferred – if not fully certified, must complete full certification within 5 years of hire date. Must submit annual personnel file updates documenting progress toward full accreditation.
• Knowledge of a variety of Mental Health Evidence-Based Interventions.
• Knowledge of multiple support systems for mental health and substance abuse clients.
• Ability to work independently and as part of a team.
• Must possess a valid driver’s license with an acceptable driving record.

Scope:

• Frequent talking and listening.
• Requires the ability to lift, carry, fold, kneel, reach, stack, stoop, and bend, which may include direct intervention with residents.
• Must be able to assist in the evacuation of clients during emergencies
