Job summary

CODA Beaverton Recovery Center is seeking an experienced Case Manager to support residential patients in Beaverton, OR. You will assess needs, remove barriers to care, and craft strong aftercare plans.

You’ll connect patients with housing, employment, public benefits, and recovery resources, coordinating with clinical staff and community partners to ensure smooth transitions and access to services. A high school diploma is required, with preference for 1 year in case management or related work;

Qualifications

• High school diploma or GED required.
• 1 year of case management, peer support, SUD services, or social services preferred.
• Familiarity with Social Determinants of Health and trauma-informed care.
• Strong interpersonal, advocacy, and communication skills.
• Ability to maintain professional boundaries while providing high-touch, compassionate support.
• Cultural humility and sensitivity with diverse patient populations.
• Proficiency in Microsoft Office and electronic medical record systems.
• Valid Oregon or Washington driver’s license.

Responsibilities

• Collaborate with clinical and medical teams to support patient treatment goals.
• Conduct non-clinical assessments to identify recovery barriers related to housing, food, employment, and more.
• Use motivational interviewing to develop patient-centered action plans.
• Attend care coordination meetings and monitor patient progress.
• Act as liaison with community partners, healthcare providers, and legal supports.
• Assist patients in accessing public benefits such as SNAP, OHP, SSI, and ID documentation.
• Maintain up-to-date knowledge of community resources and recovery services.
• Develop and maintain personalized service and discharge plans.
• Coordinate care transitions into outpatient, sober living, or higher-level care settings.
• Facilitate telehealth, dental, mental health, and medical appointments as needed.
• Ensure all patient interactions are accurately documented in the electronic health record.
