Bluegrass Care Navigators is seeking a PACE Activities Assistant to plan, coordinate, and implement therapeutic, social, and recreational activities for participants in our PACE program. You will work with the Interdisciplinary Team to promote well-being and independence in a safe, inclusive setting.
The role requires a high school diploma, relevant experience with older adults, CPR/First Aid, and a valid driver’s license; schedule is Monday–Friday, 8:00 a.m.
As a Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home. Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services. The PACE Activities Assistant supports the planning, coordination, and implementation of therapeutic, social, and recreational activities for participants enrolled in the Program of All-Inclusive Care for the Elderly (PACE). This role promotes participants’ physical, cognitive, emotional, and social well-being through engaging designed to enhance quality of life, encourage independence, and support individualized care goals. The Activities Assistant works closely with the Interdisciplinary Team (IDT) to provide participant-centered programming in a safe, supportive, and inclusive environment.
