Job summary

Bgcarenav is seeking a Home Care Certified Nursing Assistant to deliver direct personal care in participants' homes, focusing on one-to-one support, daily living activities, light housekeeping and meal preparation. You will closely observe participant status and communicate changes to the care team.

Under the supervision of an RN, you will report to the Interdisciplinary Team and coordinate care while maintaining independent field work and strong communication with colleagues and families.

Qualifications

• High school diploma or GED; must be at least 18.
• Current KY SRNA certification.
• Completed approved CNA training program.
• Minimum 1 year of experience with frail/elderly or training provided upon hire.
• Food handling permit required.
• CPR certification must be current.
• Licensed driver.
• Ability to work independently and adapt to changing participant needs and schedules.
• Strong communication with participants and interdisciplinary team.
• Ability to complete accurate documentation and records.

Responsibilities

Position Summary:

The PACE Home Care Certified Nursing Assistant (CNA) provides direct personal care and supportive services to participants in their homes, supporting independence, safety, and quality of life. This role focuses on delivering one-one-one care in a home setting, including assistance with activities of daily living, light housekeeping, meal preparation, and observation of participant condition.

Working under the supervision of a Registered Nurse (RN), the Home Care CNA plays a key role in identifying and reporting changes in participants’ physical, mental, or environmental status to ensure timely intervention and continuity of care. The position requires the ability to work independently in the field while maintaining strong communication with the Interdisciplinary Team (IDT).

Position Qualifications:

• High school diploma or General Education Degree (GED) required. Must be at least 18 years of age.
• Current State Registered Nurse Aide (SRNA) certification through the Kentucky Board of Nursing required.
• Completion of an approved Certified Nursing Assistant (CNA) training program required.

• Minimum of one (1) year of experience working with frail and elderly population or must receive appropriate training upon hire.
• Food handling permit required (to be obtained upon hire).
• Maintains current CPR certification.
• Ability to work independently with minimal supervision and adapt to changing participant needs and schedules.
• Demonstrated ability to provide compassionate care and effectively communicate with the adult and elderly population.
• Ability to read, understand, and follow written and verbal instructions, policies, and safety procedures.
