Job summary

Pathways Healthcare in Stoneham, MA seeks a dedicated Registered Nurse to join our home health and hospice team. You will assess patients, develop plans of care, administer treatments, and coordinate with families and physicians to support comfort and recovery at home.

This full-time weekend schedule (Fri/Sat/Sun or Sat/Sun/Mon for 30 hours) offers a supportive culture, ongoing education, mileage reimbursement, and a comprehensive benefits package.

Qualifications

• Licensed as a Registered Nurse in the State of MA.
• Preferably 2+ years of nursing experience with a focus on geriatrics.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Assesses and evaluates patient status by writing and initiating plan of care, regularly re-evaluating needs and revising the plan as necessary.
• Develops a care plan with goals based on nursing diagnosis and includes patient and family in planning.

Position Details:

• Service Area: Stoneham, MA and surrounding areas
• Job Type: Full-time
• Schedule: Fri/Sat/Sun or Sat/Sun/Mon - 30hours per weekend - 3 ten-hour days.

Responsibilities:

Patient Care:

• Assesses and evaluates patient’s status by:
• • Writing and initiating plan of care.
  • Regularly re-evaluating patient and family/caregiver needs.
  • Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care give
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.

Additional Duties:

• Participates in on-call duties.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfills the obligation of requested and/or accepted case assignments.
• Actively participates in quality assessment performance improvement teams and activities.
