Job summary

Pathways Healthcare is seeking a dedicated RN to join our home health and hospice team in Massachusetts. This full-time role involves delivering patient-centered in-home nursing care, focusing on geriatrics, with an emphasis on dignity and quality of life in a home setting.

You will assess patients, develop and adjust care plans with families, administer medications as prescribed, and maintain thorough clinical documentation.

Qualifications

  • Licensed as a Registered Nurse in MA.
  • 2+ years nursing experience, geriatrics preferred.
  • New nurses encouraged; exceptional training program offered.
  • Excellent bedside manners and exceptional communication skills.
  • Knowledge of healthcare laws and regulations.

Responsibilities

  • Assess and evaluate patient status; write and initiate plan of care.
  • Regularly re-evaluate patient and family needs; revise plan as necessary.
  • Develop care plans with goals based on nursing diagnosis and involve patient/family.
  • Initiate preventive and rehabilitative nursing procedures; administer meds per physician's plan.
  • Complete, maintain and submit accurate clinical notes; ensure patient safety.

Responsibilities:

  • Assesses and evaluates patient’s status by:
  • Writing and initiating plan of care.
  • Regularly re-evaluating patient and family/caregiver needs.
  • Participating in revising the plan of care as necessary.
  • Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
  • Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care give
  • Provides and maintains a safe environment for the patient.
  • Attends interdisciplinary group meetings.

Additional Duties:

  • Participates in on-call duties.
  • Ensures that arrangements for equipment and other necessary items and services are available.
  • Supervises ancillary personnel and delegates responsibilities when required.
  • Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
  • Fulfills the obligation of requested and/or accepted case assignments.
  • Actively participates in quality assessment performance improvement teams and activities.

Qualifications:

  • Licensed as a Registered Nurse in the State of MA.
  • Preferably 2+ years of nursing experience, with a focus on geriatrics.
  • New nurses are encouraged to apply; we offer an exceptional training program.
  • Excellent bedside manners and exceptional communication skills.
  • Working knowledge of healthcare laws and regulations.
  • Maintain current BLS CPR certification.
  • Possess a valid driver's license and own a reliable, insured vehicle.
