Job summary

Baton Rouge General Medical Center in Baton Rouge, LA is seeking an experienced addiction counselor to support residential and Intensive Outpatient Program patients. You will deliver evidence-based counseling, conduct psychosocial assessments, and help with treatment planning and discharge coordination.

You will identify risk factors, facilitate individual and group sessions, coordinate resources, and collaborate with a multidisciplinary team to support recovery and continuity of care.

Qualifications

• Master's degree in Counseling, Social Work, Marriage and Family Therapy or related behavioral health field.
• Minimum of 24 months experience with clients who have addictive disorders or specialized addiction counseling training.
• Current Louisiana LMHP licensure (LPC, LCSW, LMFT, LAC, or LRC) OR PLPC, LMSW, RAC, CAC, and/or CIT under supervision as required.
• Strong assessment, treatment planning, documentation, communication, crisis intervention, and interdisciplinary collaboration skills.

Responsibilities

• Provide evidence-based addiction counseling services for residential and/or Intensive Outpatient Program (IOP) patients; support recovery, treatment planning, and discharge coordination.
• Complete comprehensive psychosocial assessments and communicate findings to the healthcare team, patients, and families.
• Identify and respond to abuse, neglect, suicidal risk, and homicidal risk per regulations.
• Facilitate individual, family, and group counseling sessions as ordered.
• Serve as primary counselor for up to 10-12 residential patients or 20 IOP clients.
• Develop, monitor, and update treatment plans; document assessments, progress notes, and discharge plans.
• Participate in interdisciplinary treatment planning, care conferences, and discharge planning activities.
• Coordinate community resources and referrals to support transitions of care.
• Provide supervision to unlicensed or inexperienced professionals when licensed and authorized.
• Maintain HIPAA compliance and all organizational policies.

• Facilitate individual, family, and group counseling sessions as ordered and scheduled.
• Serve as primary counselor for up to 10-12 residential patients or 20 IOP clients.
• Develop, monitor, and update treatment plans while documenting assessments, progress notes, and discharge plans.
• Participate in interdisciplinary treatment planning, care conferences, and discharge planning activities.
• Advocate for patients throughout the treatment process and encourage patient and family involvement in care decisions.
• Coordinate community resources and referrals to support successful transitions of care.
• Provide supervision to unlicensed or inexperienced professionals when licensed and authorized.
• Maintain compliance with HIPAA, safety requirements, and all organizational policies and procedures.
• Perform all other duties as assigned.
