Job summary

Brooks Bartram Crossing in Jacksonville, FL seeks a PRN Physical Therapist Assistant to administer PT treatments under the supervision of a Physical Therapist. The role supports discharge planning and patient education at a high-performing skilled nursing facility.

You will review records, help develop and adjust care plans, document progress, and collaborate with therapists and other clinicians to meet patient goals. Florida PTA license and CPR/BLS are required; new grads considered.

Qualifications

• Current Florida PTA license.
• CPR/BLS certification required.
• Experience preferred; new grads considered.
• Knowledge of federal and state professional requirements.

Responsibilities

• Administers physical therapy treatments under the direction of and as an assistant to the Physical Therapist.
• Reviews patient records for physician orders and changes in patient status.
• Updates and adjusts treatment plan with input from the Physical Therapist and patient/family needs.
• Contributes to discharge planning and writing home exercise programs as appropriate.
• Maintains accurate documentation of progress notes, scores, and education per policy.
• Attends staff meetings and in-services; supports departmental productivity.
