Job summary

Humana Inc. seeks an Associate Vice President, Care Management to provide strategic leadership for Indiana Medicaid care coordination, LTSS, HCBS, and population health operations.

The role requires integration of clinical, operational, and regulatory considerations, with accountability for program performance and outcomes across multiple care platforms. The AVP will guide teams, ensure compliance, and drive initiatives that improve member experience and transitions of care, reporting directly

Qualifications

• Must reside or relocate to Indiana and hold one of the required licenses.
• 10+ years of progressive experience in managed care, Medicaid, care management, or related healthcare operations.
• 8+ years of leadership experience directing multidisciplinary teams.
• Experience directing large-scale care management, care coordination, LTSS, HCBS programs.
• Knowledge of Medicaid managed care requirements and audit readiness.
• Ability to establish operating models, staffing plans, and performance metrics.
• Strong analytical and financial acumen with data-driven decision making.
• Proficiency with Microsoft Office applications and enterprise reporting tools.
• Commitment to improving member experience, health outcomes, and quality of life.

Responsibilities

• Provide executive leadership for care coordination and service delivery across LTSS, HCBS, and Medicare/Medicaid programs.
• Translate contract requirements and strategy into reliable operating models with measurable outcomes.
• Lead cross-functional teams and ensure regulatory compliance and audit readiness.
• Oversee staffing models, workforce planning, and performance monitoring.
• Partner with providers, regulators, and internal teams to drive program improvements.
