Job summary

Elara Caring is seeking a compassionate Home Health Physical Therapist to join our Worcester team. You will deliver personalized, one-on-one home-based physical therapy to help patients improve mobility, safety, and independence while managing treatment plans with support from an experienced interdisciplinary team.

You will evaluate patients in their homes, develop plans of care, educate families, monitor progress, and adapt therapies as needed.

Qualifications

• Graduate of a physical therapy program accredited by CAPTE.
• Current, unrestricted Physical Therapist license in Massachusetts.
• Current CPR certification through AHA or Red Cross.
• At least one year of Physical Therapist experience in a healthcare setting preferred.
• Home health experience preferred but not required.

Responsibilities

• Evaluate patient functional abilities and develop individualized plans of care to improve safety, mobility and independence.
• Perform comprehensive home-based physical therapy evaluations and document progress.
• Educate patients and caregivers on exercises, fall prevention, safety, and use of assistive devices.
• Collaborate with nurses, physicians, OTs, SLPs and therapy assistants to coordinate care.
• Supervise PTAs and support staff when applicable and ensure compliance with HIPAA and agency policies.

Physical Therapist degree (DPT or equivalent)
