Job summary
Residential Home Health and Hospice in Marion, IL is seeking an Occupational Therapist to provide skilled home health care to patients in their homes within the assigned territory. You will perform ADL-focused evaluations and develop treatment plans to promote independence and safety.

Qualifications
- Active Occupational Therapist, OT license in the state you work
- One year of experience as an Occupational Therapist
- A valid driver’s license and ability to drive ~20% of workday
Responsibilities
- Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team
- Perform comprehensive patient evaluations focused on ADLs, upper extremity function, and home safety
- Develop individualized treatment plans to promote independence and quality of life
- Educate and empower patients and caregivers on adaptive techniques and home safety
- Ensure thorough, compliant EMR documentation
- Travel to patient homes within assigned territory
