Job summary

WellBe Senior Medical is seeking a Nurse Practitioner (APP) to provide comprehensive primary care in patients' homes in Columbus, OH. This field-based role emphasizes autonomy, collaboration with an interdisciplinary team, and a mission to prevent ER visits through proactive, holistic care.

The position offers a substantial sign-on bonus, mileage reimbursement, full benefits, and relocation support. Licensure as NP (ANCC/AANP) and an active credential are required to deliver high-quality care

Qualifications

• Master’s Degree (NP) with focus on Adult, Ger iatric, Acute Nurse Practitioner, and/or Family.
• Active, unencumbered NP license and Board Certification (ANCC or AANP).
• Reliable mobility: valid driver’s license and vehicle for local field-based travel.

Responsibilities

• Deliver Field-Based Care: Conduct comprehensive primary care visits and physical exams in the comfort of a patient’s home.
• Manage Chronic Disease: Create longitudinal care plans that move the needle for high-risk patients.
• Collaborate Across the Continuum: Partner with an interdisciplinary team to ensure seamless, safe care transitions.
• Prevent ER Visits: Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital.
