Job summary

Health Link Home Health & Hospice in San Francisco, CA, is seeking a Registered Nurse (RN) Home Health – Case Manager for a full-time role. The position offers hourly pay between $64.90 and $75.00, with a $10,000 sign-on bonus and a stable full-time schedule.

RNs will manage caseloads with OASIS assessments, provide skilled nursing care, coordinate with the multidisciplinary team, and educate patients to safely manage conditions at home. A California license and driver’s license are required.

Qualifications

• Current California RN license required (please include license number)
• Minimum 1 year of professional nursing experience
• Home health experience preferred
• Valid California driver's license and reliable transportation
• Strong clinical judgment, organization, and ability to work independently

Responsibilities

• Perform comprehensive OASIS assessments including Start of Care, Recertifications, and Discharges
• Administer medications, provide wound care (including wound vacs), and manage IV therapies
• Coordinate care as the primary contact for physicians and the interdisciplinary team to keep the plan of care on track
• Educate patients and families to manage conditions safely at home
• Document accurate, timely clinical notes using AI-assisted tools
