Job summary

VitalCaring Group in Missouri invites a Field-Based Home Health RN to lead plans of care, coordinate across disciplines, and drive outcomes from admission to discharge.

You will perform in-home assessments, develop care plans, deliver skilled nursing, oversee aides, educate families, and ensure timely documentation in the EMR, with on-call rotation.

This role offers clinical autonomy, a supportive team, flexible scheduling, and meaningful, measurable patient impact in patients' homes.

Qualifications

• Active RN license (or compact eligibility)
• CPR certification
• Graduate of an accredited nursing program
• Minimum 1 year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred)
• Strong clinical judgment and ability to think independently
• Excellent communication skills and patient-centered approach
• Proficiency with EMR systems
• Valid driver’s license and reliable transportation

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care
• Develop and manage individualized care plans in collaboration with physicians and care teams
• Deliver skilled nursing care, including medication management, treatments, and interventions
• Serve as the case manager, coordinating care across therapists, aides, and physicians
• Monitor patient progress and make clinical decisions based on changing conditions
• Educate patients and families to support disease management and independence
• Coordinate care transitions and discharge planning to reduce readmissions
• Maintain timely, accurate documentation in the EMR
• Supervise and support Home Health Aides as needed
• Participate in on-call rotation as appropriate
