VitalCaring is seeking a Field-Based Home Health RN to lead patient care in patients' homes within an assigned territory. You will manage plans of care, coordinate across disciplines, and drive outcomes from admission to discharge.

You will apply advanced assessment, care planning, and patient education to reduce hospitalizations and promote long-term independence at home. This role emphasizes autonomy within a collaborative clinical team.

Qualificações

• RN licensed (or compact license) with CPR certification.
• Graduate of an accredited nursing program.
• Minimum 1 year RN experience; home health, Med-Surg, ICU, or acute care preferred.
• Strong clinical judgment and ability to think independently.
• Excellent communication skills and patient-centered approach.
• Ability to manage a caseload and prioritize effectively.
• Proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsabilidades

• Perform in-home assessments to evaluate status, risks, and goals.
• Develop and manage individualized care plans with physicians and teams.
• Deliver skilled nursing care including medications, treatments, and interventions.
• Serve as case manager coordinating across therapists, aides, and physicians.
• Monitor progress and adjust care based on changing conditions.
• Educate patients and families for disease management and independence.
• Coordinate transitions and discharge planning to reduce readmissions.
• Maintain timely documentation in the EMR.
• Supervise Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
