VitalCaring Group is seeking a Field-Based Home Health RN to lead clinical care for patients in their homes. You will manage the plan of care, coordinate with physicians and interdisciplinary teams, and drive outcomes from admission through discharge.

Qualifications

• Active RN license (or compact eligibility) and CPR certification.
• Graduate of an accredited nursing program; BSN preferred.
• Minimum one year RN experience; Home Health or related settings preferred.
• Strong clinical judgment and ability to think independently.
• Excellent communication and patient-centered approach.
• Proficiency with EMR systems and documentation.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans in collaboration with physicians and care teams.
• Deliver skilled nursing care, including medication management, treatments, and interventions.
• Serve as the case manager, coordinating care across therapists, aides, and physicians.
• Monitor patient progress and make clinical decisions based on changing conditions.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate documentation in the EMR.
• Supervise and support Home Health Aides as needed.
