Job summary

Heritage Wound Management Specialists (HWMS) seeks a Wound Care Nurse Practitioner (NP) to join their mobile/home-based care team in North Carolina. You will manage your own patient panel, set your schedule around patient needs, and provide autonomous wound care in homes and facilities within the Raleigh‑Durham region.

Compensation is $110,000 to $140,000 per year, with bonus potential and mileage reimbursement. Requires NC license, NP/PA certification, and DEA license (or ability to obtain).

Qualifications

• Active, unrestricted North Carolina license.
• Board Certification (FNP, AGNP, PA-C, or similar).
• DEA license (or ability to obtain).
• Valid driver's license and reliable transportation.

Responsibilities

• Manage own patient panel and provide wound care independently with clinical autonomy.
• Coordinate care with home health agencies, SNFs, and referring providers.
• Ensure Medicare and payer documentation compliance.
• Document accurately and timely following HWMS protocols and workflows.
