Job summary

Unicity Care Management is seeking a Licensed Social Worker to lead a private-pay, in-home care management caseload. This field-based role offers autonomy, scheduling flexibility, and direct client relationships without discharge queues or dashboards.

You will coordinate health care decisions, advocate for families, and travel to client homes as needed, providing high-touch support and crisis intervention when required.

Qualifications

• Licensed Social Worker (LSW or LCSW) licensure required.
• Ability to conduct in-home assessments and document findings.
• Willingness to travel for field visits.
• Strong written and verbal communication under stress.
• Proven organizational discipline for independent caseload management.

Responsibilities

• Conduct comprehensive in-home assessments covering functional, cognitive, psychosocial, and environmental needs.
• Develop and maintain individualized care plans, revising them as circumstances change.
• Coordinate physicians, home care, specialists, and community resources into a single coherent plan.
• Support clients and families through transitions of care and close care gaps.
• Provide emotional support and crisis intervention when needed.
• Advocate for clients at appointments and care conferences.
• Serve as primary contact for the caseload, including urgent needs outside normal hours.
• Field-based role with regular travel and concierge-level nursing support as needed.
