Job summary

FOX Rehabilitation is expanding in Downingtown, PA, and seeks licensed Speech Language Pathologists to deliver one-on-one geriatric care in homes and senior living communities. You will assess, plan, and treat to improve mobility, independence, and life quality, with the option to incorporate telehealth as needed.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Speech Language Pathology program.
• Current CPR certification and CCC credential.
• Basic computer and EMR proficiency.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and functional independence.
• Document in FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
