Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to provide one-on-one care in Daphne, AL, delivering in-home or senior living community services under the Geriatric House Calls model.

You will perform evaluations, develop care plans, and deliver evidence-based therapy to older adults, with autonomy over your schedule and opportunities for telehealth where appropriate. Join a clinician-led team committed to quality outcomes and aging in place.

Qualifications

• Valid Speech Language Pathology license in the state of practice.
• Degree from an accredited Speech Language Pathology program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC).

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults wherever they call home.
• Perform evaluations, develop individualized plans of care, and deliver treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and independence.
• Document effectively within FOX’s EMR system according to standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
