Job summary

Mosaic in Pontiac, IL is seeking a dedicated Registered Nurse (RN) to oversee health and well-being in community residential homes across Pontiac and Dwight. The role emphasizes autonomy, meaningful relationships, and person-centered care in a warm, home-like setting.

You will perform health assessments, collaborate with clinicians, educate staff on care protocols, and ensure precise documentation. The position offers a daytime schedule (8:00 AM – 4:30 PM, M–F) with shared after-hours coverage.

Qualifications

• Active Registered Nurse license in Illinois.
• Minimum of 1 year of nursing experience.
• ASN or BSN preferred.
• CPR & First Aid certification required; Illinois RN-T Rule 116 within 6 months.

Responsibilities

• Perform monthly health assessments and monitor condition changes.
• Collaborate with doctors, specialists, and virtual care.
• Educate and empower direct support staff on health procedures.
• Advocate for person-centered health decisions and accompany to appointments.
• Maintain accurate medical notes in Therap EHR.
