Job summary

Mosaic is seeking a dedicated Registered Nurse (RN) to oversee health and well-being for individuals with IDD across community residential homes in Pontiac and Dwight, IL. You’ll work in a home-like environment, promoting autonomy, meaningful relationships, and person-centered care.

As part of a supportive local and national nursing network, you’ll conduct health assessments, collaborate with physicians and telehealth, and educate staff on medications and health protocols.

Qualifications

• Active Registered Nurse (RN) license in Illinois.
• Minimum of 1 year of general nursing experience in community/home health or IDD setting is a plus.
• ASN or BSN degree preferred.
• CPR & First Aid certification (or willingness to obtain) and Illinois RN-T Rule 116 certification within 6 months of hire.

Responsibilities

• Perform monthly health assessments and monitor changes in condition.
• Collaborate with doctors, specialists, and StationMD virtual care to advocate for optimal health outcomes.
• Educate and empower direct support staff on medication administration and health protocols.
• Accompany individuals to medical appointments and maintain accurate notes in Therap EHR system.
