Job summary
Mosaic is seeking a dedicated Registered Nurse to oversee health and well-being in Pontiac and Dwight community homes. You will provide person-centered health oversight, collaborate with physicians and virtual care, and mentor staff in medications and protocols within a home-like setting.
The role emphasizes autonomy, meaningful relationships with individuals served, and a balanced daytime schedule. IL RN license and CPR/First Aid are required; a BSN/ASN is preferred.
Qualifications
- Active RN license in Illinois.
- Minimum 1 year nursing experience; IDD/home health experience a bonus.
- ASN or BSN preferred.
- CPR/First Aid certification or willingness to obtain.
- Valid driver’s license in good standing.
Responsibilities
- Oversee health and wellbeing across residential homes in Pontiac and Dwight.
- Perform monthly health assessments and review physician orders.
- Collaborate with doctors and virtual care partners to optimize health outcomes.
- Educate and empower direct support staff on medications and health protocols.
- Document accurately in Therap EHR and coordinate medical appointments.
