Job summary

UC San Diego Health in La Jolla is hiring a CLIN NURSE 2 for the Pulmonary Specialty Clinic. The RN coordinates outpatient clinic care for patients with advanced pulmonary diseases, providing triage, referrals, education, and care coordination for complex cases.

They collaborate with physicians and the care team to ensure access to medications, DME, and services in a busy high-quality setting. The role requires autonomy, strong nursing judgment, and effective communication with diverse patients

Qualifications

• Must be a graduate from an accredited school of Nursing.
• RN License issued by the State of California.
• BLS/CPR at time of hire with commitment to get BART within six months.
• Three years of directly related experience in a large complex healthcare or clinic setting.
• One year of nursing experience in ambulatory care within the past five years.
• Knowledge of pulmonary physiology and complex chronic disease management.
• Experience with patient triage, referrals, and care coordination.
• Ability to provide complex patient education and independent nursing judgments.

Responsibilities

• Triage by telephone, MyChart, and in-person; manage provider inbox coverage.
• Schedule referrals and obtain outside medical records; coordinate medications and DME.
• Coordinate prior authorizations and patient education activities.
• Collaborate with multidisciplinary teams to ensure timely access to care.
• Maintain accurate records and navigate insurance and authorization processes.
