Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist Assistant to provide individualized care to older adults in Assisted Living Facilities and in home/community visits. The PTA will manage a full caseload, exercise clinical autonomy, and collaborate with experienced therapists to help patients stay active, independent, and aging safely at home.

• Strong documentation within 48 hours.

Responsibilities

• Manage a full caseload and provide individualized treatments.
• Primarily treat patients in ALFs, with occasional home/community visits.
• Manage your patient schedule within the 9:00 AM–5:30 PM workday.
• Communicate with patients, families, caregivers, and clinical staff.
• Exercise clinical autonomy while collaborating with experienced therapists.
• Complete documentation within 48 hours.
