Job summary

Chautauqua Health & Rehabilitation seeks a licensed Physical Therapist to provide therapy and related services. You will develop treatment plans, supervise PTAs and rehab techs, and coordinate discharge planning in collaboration with the care team.

You will maintain professional standards, ensure compliant documentation, and advance clinical expertise through ongoing education while serving a diverse patient population in a facility setting.

Qualifications

• Graduate of an APTA-accredited school of physical therapy.
• Valid state Physical Therapist license.
• Proficiency with Microsoft Office applications and basic internet navigation.

Responsibilities

• Provide physical therapy and related services for patients while maintaining positive interactions with facilities and clients.
• Develop and implement initial and ongoing plans of care and discharge planning.
• Supervise assistants and rehab techs, and participate in case management responsibilities.
• Maintain professional conduct and adhere to regulations governing physical therapy practice.
• Ensure compliance with documentation and billing guidelines in the medical record.
• Foster interdisciplinary collaboration and ongoing professional development.
